NCCAOM Handbooks with Applications
As a service to our candidates, the NCCAOM® Certification and Recertification Handbooks, which include the application form may be downloaded free of charge. (Adobe Reader required)
The NCCAOM®Certification Handbook is the official source of information for applicants seeking certification(s) with NCCAOM. It contains details about meeting eligibility requirements and testing, as well as an explanation of the difference between State Licensure vs. NCCAOM Certification. The NCCAOM application form for certification is found at the back of the NCCAOM® Certification Handbook. The certification application is valid for four (4) years from the date NCCAOM receives the application.
In order to maintain “Active” certification status, Diplomates must recertify every four years. In order to recertify, the Diplomate must obtain CPR certification and complete a minimum of 60 PDA points during the four year period prior to the certificate’s expiration date. Visit the NCCAOM PDA Search Engine for a quick and easy reference to NCCAOM approved PDA providers.

The NCCAOM School Code List was developed by our staff for the purpose of tracking, by institution and exam registration with Pearson VUE. You will be asked for your school code when registering with Pearson VUE for the NCCAOM examinations. An identifying numeric value is assigned to schools which have multiple applicants apply to NCCAOM through any of the eligibility routes. In order to ensure timely and accurate processing of applications, please refer to the school list attached when completing section I in the Application for Certification. The school code is the alpha-numeric (ABC-12345) value listed after the school name.
