CONTINUOUS STATE LICENSE REQUIREMENT
An applicant for the Reinstatement must have held a continuous active state license to practice acupuncture since the applicant’s certification expiration date. If candidate had practiced in more than one state over the years, copies of all licenses have to be reported during the reinstatement application submission. Please note: The following documents are required in electronic format during the reinstatement application submission:
- A copy of each state’s Acupuncture license(s) since the applicant’s certification expiration date;
- Proof of the most current free and clear Acupuncture state license.
The NCCAOM® Reinstatement to Active Certification Application is located in the former Diplomate’s online account. The application is valid for twelve (12) months from the date of submission. All reinstatement requirements must be met within the six 12-month time period, or a new application must be submitted. when the original application expires.
