NCCAOM PDA Provider Application
Step 1: Submit the application
To apply, go to the NCCAOM website at www.nccaom.org. At the top right corner, click ‘Login’ and then enter the PDA Provider portal. Click on create an account and enter your information. Once the PDA Provider account is created, complete the NCCAOM PDA Provider Application.
A $350 one-time initial Provider payment is due at the time the application is submitted for a life-time membership. Active Diplomates receive an automatic 10% discount and 501(c)3 non-profit organizations registered with the PDA Department get a 50% discount.
The approval process takes two to three-weeks.
